Please have your driver’s license or state identification card and Social Security number ready at the time of your appointment as your identity will need to be verified. If you are a POA or guardian representing a claimant, you must present the appropriate legal documents at the time of the appointment.

- The following
situations require additional investigation by staff. Please make sure you have responded to fact
finding sent by the agency through your MiWAM account. These items will not be resolved during your
appointment time.

- Additional claim indicators
- Appointments do not include the option of filing new claims
- This appointment is not transferrable to another person.
- Appointments can be scheduled up to 7 days in advance. If no appointments are available in the upcoming week, you'll see a message indicating the schedule is full and times are not available well in advance of today's date. Please continue to check back to try to schedule an appointment at a later time. - If you no longer need your scheduled appointment, please cancel it to allow someone else to schedule the time.
